Health Care: The Heart Of What We Do
 As a part of United Church Home's family of senior living communities, Patriot Ridge Community places great importance on exceptional rehabilitation and nursing services. Our trained staff works with families and physicians to develop plans of care to support each resident's abilities and interests. These plans may include specialty medical care; physical, occupational, speech or respiratory therapy; day-to-day assistance with dining, hygiene, recreation; and other individual and social activities.
To improve the care and satisfaction of our residents, we are the only facility in the area to have implemented the Comprehensive Disease Management System (COMS™). This detailed system allows us to better monitor our residents’ health and identify any changes of condition earlier. When we are able to intervene earlier, we are often able to prevent the need for an unnecessary trip to the hospital.
